Families Hoping to Adopt

The ACC Parent Profile and forums are available only to those that have been approved by The Adoption Center of Choice. In order for your Parent Profile to go live your profile must be approved by The Adoption Center of Choice. The Adoption Center of Choice must also have in its possession the potential adoptive families home study and background check.

1. I understand that I am not at this time obligating myself to use the Profile Builder Tools, the content and the other resources that are provided on the secure servers of The Adoption Center of Choice, to assist users in entering information that can ultimately be used to create a ACC Parent Profile, or to pay for any services. I also understand that I am not requesting or obligating The Adoption Center of Choice, or Blue Sky Marketing Choice, L.L.C. to do anything for me, other than to consider granting me a non-transferable and revocable limited use license to personally view and use the ACC Profile Builder Tools.

   2. I understand that if I am granted the requested limited use license, it may not be used for any purpose other than to jointly create a Parent Profile for myself and my Husband or Wife, in the manner provided in the Eligibility Requirements. I acknowledge that the limited use license does not include the right to copy, download, transmit, share or republish any of the information or materials that I will have access to through the use of my limited use license, nor will I allow others to use this license (or my associated user name or password) in any way, including to view or use any of the resources that I can gain access to through the use of that license.

   3. I also understand that if I am granted the requested limited use license, it will be non-transferable, its use will be a privilege and not a right, and it may be revoked at any time, for any reason or for no reason at all, at the sole discretion of either The Adoption Center of Choice, and without any prior notice to anyone.

   4. I certify that all information I will provide in my request for and use of this limited use license will be true, correct and complete, and agree that if I obtain or attempt to obtain a license based on information or an intent that is false or misleading, that the obtained license will be void and I will not use it nor allow others to use it in any way.

I agree to be bound by all of the foregoing Terms and Conditions for Receiving a Limited Use License and want to proceed with this process.
